Standard Questionnaire Content

Your ContractorXchange Questionnaire includes 41 mandatory core questions across key areas:

  • Contact Information

  • Organization & Capacity

  • Legal

  • Occupational Health & Safety (OHS) Management

  • H&S Performance

  • Regulatory Compliance

  • Quality Management

  • Contractor Management

Client Added Questions

Each client can customize their questionnaire by selecting from 400+ additional questions.
These customizations are set during account configuration with that client.

Complete Questionnaire Page

To review your complete questionnaire:

  1. Go to Complete Questionnaire.

  2. View your Total Progress and Progress per Client.

  3. If a client appears here, they’ve added questions for you to answer.

Who Asked This Question?

Each question shows who added it under “Included by...”

  • Required for Approval: Must be completed to maintain or gain approval.

  • Optional Questions: Not required, but recommended for stronger approval status.

Managing Outstanding Questions

When you connect with a new client, check your Questionnaire Completeness widget. If your completion percentage changes under:

  • Required for Approval by All Clients, or

  • Requested by All Clients,
    ​…it means new questions were added.

To find and complete them:

  1. Open the Questionnaire widget.

  2. Click Outstanding Questions.

  3. Set Show to All Outstanding Questions.

  4. Click View to go directly to page with the incomplete question.

  5. Make sure to click on Save and Proceed at the bottom of the page
